Forms/Questionnaires

This practice uses web-based forms to collect information from you. The contact form native to Squarespace can be used for initial contact, but please note that it is not HIPAA-compliant and should not be used to enter personal health information. Please use the forms on my contact page for HIPAA-secure privacy.

Client forms embedded on my contact page, are HIPAA-compliant and can be used to communicate personal/protected health information. These forms include contact forms, feedback forms, informed consent agreements, therapy-related information forms, authorization forms, assessment and evaluation forms, financial and legal agreement forms, and other forms necessary for conducting the business of this company. These forms are subject to the Squarespace privacy policies and terms of service

This site contains embedded YouTube videos. Please note that YouTube is a Google product that collects tracking data and personal data for every YouTube video viewed.

Therapy client data containing protected health information will be retained per the company’s records storage policies and procedures.
